6 months with 3 month early out

Comprehensive Start-Up Package

Basic Services (included in all consulting packages)

• All travel-related expenses for onsite consulting, billed at cost plus 15% administrative fee
• No maximum number of site visit requests
• Onsite preceptorship by company physician, minimum 3 days
• Onsite per diem for onsite non-physician visits
• Early Out - written notification by either party within 30 days prior to early out period
• Initial site assessment by consulting team leader for up to 5 days maximum for needs assessment and verification of client expectations (after contract signing)
• Weekly conference call with team leader and hospital liaison to address issues and provide direction
• Telephonic or e-mail access to various team members throughout term

Optional Services (available at an additional cost)

• Atlanta physician preceptorship (3 days)
• Onsite physician preceptorship at opening (5 days)
• Physician support retainer, month-to-month following term
